2009 APPLICATION FOR CONCESSION SPACE

An application is required each year regardless of previous participation.  THIS IS STRICTLY AN APPLICATION.  The form is NOT a commitment by the National City® Autumn Leaf Festival™ Committee to rent space.  DO NOT send money with application.  Completely fill out ALL THREE (3) pages of this information sheet.  This application will not be processed if any part is not completed. 

GENERAL INFORMATION
If you are accepted, you will receive an official contract along with our rules, regulations and pertinent festival information. The National City® Autumn Leaf Festival™ Committee reserves the right to cancel a contract at any time for any reason. 

MISCELLANEOUS
Failure to comply with any rule or regulation, policy, or procedure of the National City® Autumn Leaf Festival™ Committee will result in forfeiture of all premiums and rights without recourse.
All accompanying rules, regulations and guidelines of the National City® Autumn Leaf Festival™ Committee are part of the contract. Therefore, any violation of the terms in these statements is considered a breach of contract.
The National City® Autumn Leaf Festival™ Committee reserves the right to rule on any question that is not addressed in the contract.
There is no agreement or options to renew the contract for any subsequent festival and no renewal or claim of renewal shall be valid.
